2) XLNT Brain Concussion Test
3) Medical Release Form
4) Code of Conduct Read and Filled out.
5) $40 Activity Card, ( Free Admission to all HS & MS Regular Season Games)
All forms can be found at www.nppsd.org web site under the Adams tab. The Office at Adams will be open to hand in forms, get forms and get activity Cards starting Monday August 1st from from 8:00am- 3:30 pm. Schedule Physicals as Dr. Office fill up.
