2010-2011 KINDERGARTEN PARENTS/GUARDIANS
Nebraska law requires that your child must have 1.) updated immunizations 2.) health physical 3.) vision exam BEFORE they can start kindergarten. A complete immunization record with dates of all immunizations and a physical and vision form, filled out by your healthcare provider, MUST be turned in by the first day of school. If the school does not have all of the needed information, your child will not be able to attend school until the requirements are met.
You will be notified by letter in April or May as to what your child needs in order to meet the state requirements.
All students entering 7th grade for the 2010-2011 school year must have a 7th grade physical and updated immunizations.
A new law, effective in July 2010 requires that all 7th graders have 1 dose of Tdap vaccine which must contain pertussis booster. The law requires that this shot be given after age 10 or 11 depending on which brand of vaccine is given. The physical form and updated immunization record must be turned into the school office before the first day of school or your student will not be allowed to attend until we have the needed information.
